Florida’s Agency for Health Care Administration selected Donate Life Florida as the contracted vendor to create the Joshua Abbott Organ and Tissue Donor Registry. Donate Life Florida is a coalition of the state’s organ, tissue and eye donor programs.
Donate Life Florida adheres to the strictest and most up-to-date guidelines to keep all personal information confidential. Aside from standard information such as name and address, the only sensitive information we require is place of birth, while mother’s maiden name and driver license number are optional.
Florida’s organ, tissue and eye donor registry does not collect any personal information about visitors to this Site (such as names, addresses, telephone numbers or e-mail addresses) unless visitors provide such information voluntarily. If visitors do not want their personal information compiled, such information should not be submitted through this Web site. If personal information is submitted, such information may be used as follows: (a) information submitted by visitors wishing to be organ, tissue or eye donors will be added to the Joshua Abbott Organ and Tissue Donor Registry (the “Registry”) and utilized for Registry purposes as defined under Florida law; (b) information submitted by visitors may be stored and analyzed to evaluate the effectiveness of efforts made to increase the number of donors as permitted by Florida law; and (c) information submitted may be used to contact individuals and/or their family members for Registry purposes as allowed under Florida law. Donate Life Florida will not sell, rent or market personal information to third parties. All information submitted through the Registry will be kept confidential at all times, except to the extent necessary for organ, tissue and eye donations to be achieved in accordance with Florida law.
